Celiac Disease Versus Gluten Sensitivity: New Role for Genetic Testing and Fecal Antibody Testing?

Every individual has two DQ serotypes. Because the molecular HLA nomenclature can be confusing DQ serotyping is a method for simplifying the results. There are four major types and 5 subtypes: HLA DQ1, DQ2, DQ3 and DQ4; DQ1 has two subtypes; DQ5 and DQ6 whereas DQ3 has three subtypes; DQ7, DQ8 and DQ9. Each individual has two copies of HLA DQ. One DQ type is inherited from each parent. Though 35-45% of individuals of Northern European ancestry are DQ2 &/or DQ8 positive only 1% have classic CD as defined by abnormal blood tests and small intestine biopsies. Several autoimmune conditions also occur more frequently in DQ2 and DQ8 positive individuals. There is accumulating scientific evidence that many individuals are gluten sensitive and respond to a gluten free diet though they have normal blood tests and/or normal intestinal biopsies (fail to meet strict criteria for CD). This is more commonly being referred to as non-Celiac gluten sensitivity (NCGS). Many individuals who have NCGS are relatives of confirmed Celiacs and were previously referred to as latent Celiacs. Electron microscopy and immunohistochemistry studies of individuals with normal biopsies but suspected of or at risk (1st degree relatives of Celiacs) have revealed ultrastructural abnormalities of the intestine and those who chose a gluten free diet usually responded and many who did not ultimately developed abnormal biopsies on long term follow-up. Seronegative Celiac has also been recognized, that is blood tests are negative, but the biopsy reveals classic abnormalities of Celiac and the individual responds to gluten free diet. Fecal antibody testing for gliadin (AG) and tissue transglutaminase (tTG) by Enterolab in Dallas has revealed elevations in 100% of Celiacs tested and up to 60% of symptomatic individuals without Celiac disease (NCGS) even if not DQ2 or DQ8 positive. The only DQ pattern he found not associated with gluten sensitivity is DQ4/DQ4, a pattern typically found in non-Caucasians who are known to have a low prevalence of Celiac disease. Testing for DQ2/DQ8 has been suggested as a way to exclude CD. That is, if you are negative for DQ2 and DQ8, then you are very unlikely to have CD. However, well documented cases of CD and Dermatitis Herpetiformis (DH) have been confirmed in DQ2 and DQ8 negative individuals. Moreover, we now have the clinical experience that other DQ patterns predispose a person to gluten sensitivity because these individuals frequently have elevated fecal antibodies to AG or tTG and respond to a gluten free diet. Why some people develop Celiac Disease or become gluten sensitive is not well understood. Risk factors include onset of puberty, pregnancy, stress, trauma or injury, surgery, viral or bacterial infections including those of the gut, medication induced gut injury or toxicity (e.g. NSAIDs), immune suppression or autoimmune diseases, and antibiotic use resulting in altered gut flora (dysbiosis). The severity of the sensitivity is related to the DQ type, pre-existing intestinal injury, degree of exposure to gluten (how frequent and large a gluten load an individual is exposed to), and immune status. Once initiated, gluten sensitivity tends to be lifelong. True CD requires lifelong complete gluten avoidance to prevent serious complications, cancers, and early death. Serotypes can be determined from blood or buccal mucosal cells (obtained by oral swab) from several commercial labs including Prometheus, Labcorp, Quest, The Laboratories at Bonfils, and Enterolabs. Fecal IgA anti-gliadin and IgA tissue transglutaminase antibody testing is only available in the U.S. commercially through Enterolabs. The fecal AG and tTG testing may be helpful in those with normal blood tests for Celiac and/or a normal small bowel biopsy but suspected of being gluten sensitive. Hiring for Your Craft Show Business

Needless to say, the success of your crafts at craft shows will also have a considerable effect on whether or not you will need to hire employees. On the “free” end, you can get help from friends and family to produce your crafts and help with any business aspects. Many crafters have a good photographer they use for promotional photos and slides of their work. Quality slides can make the difference in getting accepted to juried craft fairs. You may also occasionally need the services of a graphic designer to create brochures, hangtags or other marketing materials and a PR person if you want to take out ads or run press releases. Auvergne Gastronomy- Hearty Cuisine - Winter Warmers For Hikers And St Nectaire-The Best Of Cheeses

By JS Selfe The Auvergne is a largely mountainous region in central France, part of the Massif Central. In summer the climate is hot and dry - ideal for walking, hiking and horse-riding, while winters are comparatively harsh - making ideal conditions for skiing. So the gastronomy of the region is geared to satisfying hearty appetites. Auvergne Cuisine Soupe de Nol is made by boiling chunks of stale crusty bread to salted water until the water has reduced, when pieces of St Nectaire cheese are added. The mixture is stirred until the cheese has melted, then thick crme fraiche is added just before serving. Truffade is a hearty dish made by cooking potato slices in duck fat before adding grated Cantal or St Nectaire cheese. Once the cheese is melted it is ready to eat. Aligot another hearty dish is creamed potato to which Cantal cheese is added. When the cheese has melted to the point where when the spoon is lifted from the pan the cheese forms stretchy strings it is ready to eat. Tripes and tripoux are delicious tripe and red wine dishes, while chou farci is cabbage stuffed with a delicious minced pork and breadcrumb stuffing. Coufidou is a wonderfully rich beef stew with an aromatic coriander seed sauce. Auvergne cheeses such as Cantal, Salers, St Nectaire, Tme d’Auvergne and Bleu d’Auvergne are renowned worldwide. However the gastronomy of the area is less well-known. St Nectaire, “the best of cheeses”, won the Sun King’s admiration when it was was introduced to the court of Louis IV the Marchal de France Henry de Sennectre. It us usually enjoyed at the end of a meal, though it is also sometimes used in salads or in tarte au Saint-Nectaire, truffade and soupe de Nol (Christmas soup). Red wine is accompanies it well; perhaps a Bourgogne or Pomerol The area of the St Nectaire AOC (appelation d’origine controle) is one of the smallest in France, within the Puy de Dme and the Cantal, the cheese taking it’s distinctive flavour from the great diversity of wild flowers and aromatic herbs to be found in the Auvergne pastures. The traditional “St Nectaire Fermier” cheese is made from raw milk twice a day, after each milking. The young cheese is made in a mold of 21cm, (13cm for the “petit St Nectaire”) wrapped in cloth and pressed for 24 hours. The cheese is washed in a saline solution and then in clear water each day for a fortnight. It is then turned regularly for the following three or four weeks. The cheese is marked with an oval plaque. The “St Nectaire Laitier” cheese is made from raw or pasturised milk. The cheese is marked with an square plaque. During the summer months of July and August many villages sre host to Marchs de Pays, when local producers set up stalls selling their produce, prepared ready to eat: aligot, truffade, charcuterie, meat dishes, cheeses, deserts, wines… so that all may compose a meal of their choice and dine in convivial company with neighbours, friends and visitors at trestle tables set up for the occasion in and around the village hall… the ideal end to a day’s outing in the Auvergne. Even if you rarely buy things for your own child, there are still birthdays and holidays which usually means your house is bursting at the seams with new toys. It’s hard to know where to put everything, and if you’re a sentimental person, it can be hard to let go of older toys to make room for new ones. To help avoid this problem, here are some easy tips to help you keep your kid’s room organized, even after birthdays and holidays. First thing’s first. You’re going to need storage. Some bedrooms don’t have closets, so you may want to buy shelves and bins. Shelves are great for storing things that you don’t want them to play with, like diapers, clothes and other baby or kid accessories. You can find all kinds of different colored bins to add to the decor of your child’s room that provide fun places to store toys. Clear bins are great because they allow your child to see which toys are in which bins. Hopefully this will prevent all the toys from being scattered all over the floor in the search for one specific toy. Putting labels on them will help you out when you need to do a quick clean up. Some kids can get quite a collection of stuffed animals going, but it can be hard to find a place to put them. When I was a kid, my mother sewed some string to the head of each animal and hung them up on my walls with push pins. If your child likes to play with them regularly, you can also purchase big Rubbermaid containers with lids at a very affordable price. Under-the-bed storage is a great alternative for very small rooms. Toys that aren’t played with as often can be stored under here. It’s also a great place to store out of season clothes. Having storage under a child’s bed can also eliminate the opportunity for them to store garbage or important school documents where you’ll never find them. You can also purchase plastic bins with several containers stacked vertically. Look for the kind with wheels and handles. This will make it easier to move it around when you’re cleaning. This will also help your child to learn organizational skills by keeping items separated in each bin. Legos in one bin, puzzles and games in another, and so on. Another great storage option are cube ottomans. They come in a variety of colors and the lid flips over to become a tray. Not only does this provide another method of storage, but seating options as well. You can also find round ottomans to add some fun style to a room without compromising space. Take your kids shopping with you and ask them to pick out some fun bins, baskets and other storage accessories. Getting them involved in organizing their own room will hopefully help them to keep it clean which will make your job a lot easier. Another tip to keep the toys at bay is to get rid of older toys every time some new ones come in. There are plenty of organizations that accept donated toys, and this could give your child a very positive feeling that they’ve done something for someone else. Any toys that are broken should be thrown away. One thing that can be hard to deal with is getting rid of toys that have sentimental value. What you have to remember is that every toy creates memories, but they will always be with you in your mind, not in the actual toy. You may want to keep a box stored away for really special toys that were gifts or that have a special story, but don’t let it become more than one box. By keeping every toy and article of clothing your child has ever received, you’re passing on these tendencies to your child. It’s much easier to avoid developing the pack-rat habit than it is to break it. In the end you’ll be glad you did, and you’ll have a much cleaner and more organized kids room to show for it!
